April 23, 2015,　Uno PhD was observed a trademark monopoly of the low -FODMAP diet (LFD) from the Japanese government. The purpose of Uno is to prevent unauthorized LFD.　Uno wants convey the correct LFD information at Japan.
The patent contents are, education of LFD, guidance of LFD, information, and dissemination of LFD.　The efficacy of this patent is limited to Japan.
In Japan, there is a unique problem. The Japanese food often contain sorbitol. For example, "Mentaiko" contains sorbitol. Also, Japanese marshmallows contains sorbitol. Japanese fruit contains fructose a lot by breeding. Component of Japanese tofu is almost the same as soy milk.

低フォドマップ食の背景

20世紀まで、過敏性腸症候群（IBS）の主な原因はストレスや自律神経異常と考えられてきました。また、いわゆる悪玉菌が悪影響しているとも言われてきました。そのため、患者さんはカウンセリングを受け、抗うつ剤を投与され、善玉菌を増やすとされるヨーグルトなどを飲むように指導されていました。

　しかし、21世紀になり、そのような治療の効果がおもわしくないことから、それらが主たる原因ではなく、何等かの食事が影響しているのではないかと考えられるようになりました。その背景には、以前から、小麦粉や牛乳など個々の食品について、IBSとの関係が問われていたこともあります。そして、IBS患者の食事と症状の関係が分析され、多くのIBSの患者では、小腸で吸収されにくい糖類などが、吸収されないまま大腸に運ばれ、大腸の腸内細菌で過剰に発酵するためではないかと考えられたのです。それを科学的に研究し、検証し、原因物質をFODMAP(フォドマップ)と定義したのが、オーストラリアの医師と栄養士でした。そして、単に1種類や2種類の食事を規制するのではなく、IBSの原因として考えられる全てのFODMAPを一定期間抑制（禁止）し、症状の軽減を確認したうえで、その後、原因物質を特定してゆく方法が低フォドマップ食として提案されました。

　一方で、アメリカを中心に、IBSのなかに、大腸の発酵菌の増加やSIBO（小腸内菌増殖症）が多いため、これらの菌を抗菌剤（リファキシミン）で減少させて、菌の異常発酵を抑制しようとする治療が同じ時期に始まりました。

それらはともに、腸内での発酵を抑止するにはどうすべきか？を考えた治療です。しかし、この考えは、20世紀の「腸内細菌を発酵させて腸内環境を整える」という概念（プロバイオティクス、プレバイオティクス）と全く相反するものであり、衝撃的なものでした。そのため、低フォドマップ食が20世紀のIBSの食事療法よりも優れているのか？さらに、低フォドマップ食では小麦粉の食品を制限するため、小麦に含まれているグルテンを摂取しない要素が含まれており、低フォドマップ食の効果というのは「グルテンフリー食」の効果なのではないか？など、さまざまな問題が浮上しました。それらに対し、オーストラリアの研究グループを中心に検証が行われ、低フォドマップ食がプロバイオティクスや20世紀の食事療法、さらに、グルテンフリー食と比して同等以上の有効性があることが証明されてきました。

FODMAPとは？

　FODMAP（フォドマップ）とは、以下の意味です。

F→fermentable 発酵性

O→oligosaccharides オリゴ糖

D→disaccharides 二糖類

M→monosaccharaides 単糖類

A → and

Ｐ →polyol ポリオール

日本人のIBSに低フォドマップ食は必要か？

　IBSでの高FODMAPの問題は、欧米だけの問題ではありません。日本人は戦前まで、乳製品や小麦粉をほとんど食

べていませんでした。しかし、戦後に急激な消費の上昇が続いています。さらに、異性化糖やキシリトール、ソルビトー

ルは、スーパーやコンビニで売られているほとんどの食品に混入されています。

　IBSは日本では戦後に症例が報告され始め、そして、今や罹患率の非常に高い疾患となりました。つまり、戦後の日本の食の変化によって、高FODMAPが増加したために、日本でIBSが増加してきた可能性があるのです。

低フォドマップ食の適応と実践方法

　この低フォドマップ食をどのような人が、行うべきであるか、そして、どのように行うかを以下に示します。

低フォドマップ食の期待される効果

これまでの研究報告では、低フォドマップ食は、IBS患者の症状を70～75％軽減することが証明されています。
